SUMMER LESSON REGISTRATION FORM

 

Please check the dates below that you wish to have a lesson.  Consider the dates carefully.  If your dates are approved,
you will be making a commitment to attend lessons on these dates.  Therefore, you will be billed in two monthly installments
for your summer lessons.  Your first payment, for 1/2 of the cost of the lessons, will be due at your first lesson of the summer.
The remaining portion will be due at the first lesson of the August.

Check only ONE day (Tuesday or Thursday) for each week (Some lessons may be substituted by a Wednesday Lesson).

Tuition Calculation Example:
Cost per Lesson Multiply by Total # of Lessons registered for Divide by 2 months = Monthly Tuition
$25 x 9 Lessons divide by 2 = $112.5/month

Cost per lesson:
30 minute lessons = $25
45 minute lessons = $37.50
1 hour lessons = $50

                                                SUMMER RECITAL

If there is enough interest, I will hold a summer recital as an incentive for studetns who work hard to finish
a book or make further progress in their current book. Please check the box if you are interested and available
to in participate in an end of the summer recital. There will be an additional fee to cover accompanist/dress
rehearsal time, which will be paid the week of the recital.

                                       PREFERRED LESSON TIMES

Lesson times will be from 12:00pm to 7:00pm. Please choose a first, second, and third choice for a lesson time.
I will atempt to accomodate your time request, however if there are conflicts with other students, your time may
need to be adjusted. Time preference is given to students currently enrolled in the studio. Please be flexible!
